Imagine taking a tablet that contains no drug at all, yet somehow, it makes you feel better afterwards. It sounds almost impossible and quite fake. If there is no medicine in the pill, how is it having an effect?
This is the strange reality of the placebo effect: the phenomenon where a person experiences a genuine change in their symptoms after receiving a treatment with no curing ingredient. And importantly, it is not simply just “imagining” feeling better. The brain can actually change the way the body processes pain and other types of symptoms.
A placebo can work partly because of expectation. If a doctor confidently tells someone that a treatment will help, the brain begins to anticipate improvement. This expectation is another factor that can active biological pathways involved in pain, stress, and reward.
One of the best-studied examples is pain. Expecting pain relief can encourage the brain to release endorphins, natural chemicals that reduce the perception of pain. Brain imaging has even shown changes in activity in regions involved in processing pain.
In other words, the tablet itself can be chemically inactive (with no medicine), but the belief surrounding it is not.
But is it all psychological? No. The placebo effect demonstrates how closely connected the brain and body really are. Expectations can influence significant physiological processes, including heart rate, hormone levels, and the perception of pain.
There is also an interesting twist: the opposite can actually happen. This is called the nocebo effect. If someone expects a treatment to cause unpleasant side effects, they may be more likely to experience them, even when the treatment itself is incapable of producing those effects.
Therefore, expectation can work in both directions.
However, the placebo effect is not a magical cure. Placebos are much more effective for symptoms that are influence by the brain, such as pain, nausea, fatigue, than for conditions where a physical cause must be directly treated. Believing that an antibiotic will work, for example, cannot make an antibiotic-resistant bacterium become sensitive to it.
This is why clinical trials use placebo groups (as I am sure many of those who study biology, have heard about these). If a new drug performs better than a placebo, researchers have stronger evidence that it is the drug itself producing the effect rather than the improvement coming from expectation, natural recovery, or any other factors involved.
Perhaps the most facilitating thing about placebos is therefore not that they “trick” the body, but that they reveal how powerful the connection between mind and biology really is.
